Exemestane

Aromasin — Suicidal Aromatase Inhibitor for Estrogen Management

What It Is

Exemestane (brand name Aromasin) is a steroidal, suicidal (irreversible) aromatase inhibitor. Unlike reversible AIs, Exemestane permanently deactivates the aromatase enzyme it binds to, meaning estrogen levels stay suppressed until the body produces new aromatase enzymes. It is the preferred AI for estrogen management during anabolic cycles.

Mechanism of Action

Irreversibly binds to and destroys the aromatase enzyme (CYP19), preventing the conversion of testosterone and other androgens into estrogen. Because the binding is permanent ("suicidal"), there is no estrogen rebound when the drug is discontinued — the body must synthesize new aromatase enzymes to restore estrogen production.

Crashed Estrogen Warning

Crashed E2 is the #1 risk with Exemestane. Symptoms include: destroyed libido, depression, dry/cracking joints, extreme fatigue, and brain fog. Start LOW and only increase based on bloodwork. Estrogen is essential — the goal is management, not elimination.

Bloodwork Required

Never dose Exemestane based on "feel" alone. Sensitive E2 bloodwork (LC/MS) is required to confirm elevated estrogen before initiating and to monitor during use. Start low, titrate based on labs.
